STEP 2: DOCUMENTS REQUIRED
Once Pre-Registration has been completed, parents MUST bring to the school office:
1. Birth certificate or other document of proof
AND
2. Current immunization record or exemption certificate
*These documents must be submitted within 30 days of the student's start date at Ephraim Elementary School!*
Upon enrollment of a student for the first time in a particular school, within 30 days parent/guardian must provide either a certified copy of the student's birth certificate, or other reliable proof of the student's identity and age, together with an affidavit explaining the inability to produce a copy of the birth certificate (53G-9-302).
The school office will make copies of these documents and secure them in your student's file.
STEP 3: POWERSCHOOL REGISTRATION
This is REQUIRED before the student starts school at Ephraim Elementary:
- PowerSchool log in page: https://ps.ssanpete.org/public/home.html
- Enter Username and Password (that you chose in pre-registration)
- Select "Online Registration" and a new window opens
- Select your student's name and complete each form
- The office will get notified when registration is complete
STEP 4: COMPLETE FREE/REDUCED MEAL APPLICATION (OPTIONAL)
The free and reduced meal application is available in your PowerSchool account. We encourage everyone to apply!

STEP 5: CLASS PLACEMENT
Registration is complete. Thank you!
We will place your student in a class and you will be able to see what teacher they have in your PowerSchool account.
